
Appointments + Referrals
If you or your child needs to see an Allied Health Therapist, then there are a number of ways that you can refer to us. You may not need a referral from a GP to see a Therapist.
Options include:
- Self-referral
- NDIS
- Medicare
- Seeking public funding
Self Referral
If you would like to self-refer, then please complete our referral form. Either in the online form below or by downloading and printing the PDF form; which can then be faxed to (07) 3354 1644, emailed to administration@ltts.com.au, or posted to PO Box 2178 Keperra QLD 4055. We will then call you to discuss the reason for your referral and to make an appointment with the appropriate Therapist. Further information will be provided at the time of this call to explain the process by which you may be able to obtain private funding services.

Public Funding
If you would like to seek public funding, then please speak to your GP first. You will need to discuss the reason why you would like a referral with your GP, and then your GP may provide you with an Enhanced Primary Care (EPC )/ Chronic Disease Management (CDM) referral. Once this GP referral has been obtained fax to (07) 3354 1644, email to administration@ltts.com.au, or post to PO Box 2178 Keperra QLD 4054. We will then call you to discuss the reason for your referral and to make an appointment with the appropriate Therapist.
